Missing Information

In general, if there is no information available to fill a particular element then it should be left empty. It should not be filled with comments or notes like "UNKNOWN", "N/A", "FLEET", etc…

Vehicle Make

This is the Vehicle Identification Number assigned to the vehicle by the manufacturer that is covered by the policy represented by the same record.

Vehicle Make

This is the name of the manufacturer (or an abbreviation of it) of the vehicle that on the same record.

Vehicle Year (YYYY)

This is the year the vehicle was manufactured.

Company Control Code

This is the "4-digit code assigned by Insure-Rite, Inc." as described above.

Policy Number

This is the insurance company’s insurance policy number.

Policy Effective Date

This date is frequently misunderstood. It does mean the earliest date at which uninterrupted, continuous, coverage started for the vehicle/driver combination specified on the record. If the policy lapsed or a vehicle was dropped from it and was later reinstated this date should be the reinstatement date of the policy or vehicle, respectively. It does not mean the earliest date at which the policy number existed.

Last Name

This is the last name of an individual or the start of a company name.

First Name

This is the first name of an individual or the remainder of a company name that would not fit in the Last Name element. The business names should start in the Last Name element and overflow into the First Name element. If there is no overflow of the Last Name element into the First Name element, leave the First Name element empty. Insure-Rite will do our best to identify these records as commercial to enable us to avoid flagging them as having incomplete First Names. Even if they are incorrectly flagged as incomplete there will be no negative consequences and the records will still be processed.

Address

This is the full street address in the mailing address on the policy.

City 

This is the city in the mailing address on the policy.

State

This is the 2-digit state abbreviation in the mailing address on the policy. It is not expected to be a number.

Zip

This is the zip in the mailing address on the policy.

Garaged Address

This is the full street address in the primary address at which the vehicle is garaged or based.

Garaged City

This is the city in the primary address at which the vehicle is garaged or based.

Garaged State

This is the 2-digit state abbreviation in the primary address at which the vehicle is garaged or based. It is not expected to be a number.

Garaged Zip

This is the zip in the mailing address on the policy.

Driver's License State

This is the 2-digit state abbreviation of the state which issued the driver license number specified in the "Driver License Number" element.

Driver's License Number

This is the driver license number, assigned by the state that issued it, to the driver named on the same record who is covered to drive the vehicle specified on the same record.

Date of Birth (MMDDYYYY)

This is the date of birth of the driver named on the same record.

Scheduled, Unscheduled or Driver only policy ("S","U", "D" or "V")
